New centers wishing to participate in the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) must submit a pre-application. The link to register a new center is located on the log in page. After submitting the pre-application, it will go through the approval process and a MBSAQIP program coordinator will contact the primary contact for the center with any questions.
Note: The pre-application does not provide a save option. It is recommended that you complete the entire pre-application at one time.
Centers renewing their participation with the MBSAQIP will have a username and password sent to the primary contact listed for the center. If the primary contact for your center has changed, please contact us at email@example.com.
Renewing centers DO NOT have to submit a pre-application. Applications for renewal are provided within the center’s profile page after logging on. Applications for renewal become available 6 months prior to the centers accreditation cycle end date.
Every center participating in the MBSAQIP is required to have a current Participation Agreement (PA) and Business Associate and Data Use Agreement (BAA/DUA) on file.
